Q. Does my spouse need to complete a webscore, Healthy Decisions counseling or wellness activities?
A. Only the employee carrying the insurance and enrolled in medical benefits through the organization are eligible for the Healthy Decisions Wellness Program. Dependents do not need to complete the wellness program requirements.
Q. I am a Healthy Decisions participant, how do I access my personal Healthy Decisions Program?
A. You will need your employee ID number to create your account. Your ID number is located on the front of your paystub. On the front page of the Healthy Decisions website (http://www.cmmfhealthydecisions.com ), under employee sign in- you will see a link to register. Click register. To register, you need to enter your employee ID number and date of birth. You will receive a confirmation email. This confirmation lets the system know that you are the right person with the correct email address. You will receive a confirmation code. The code will allow you to complete your registration and access your personal Healthy Decisions Wellness account.
Q. What is my Personal Wellness Plan?
A. Your Personal Wellness Plan is a plan describing your wellness goals, readiness to undertake change and strategies for accomplishing your health goals.
Q. Where do I find the Personal Wellness Plan?
A. The Personal Wellness Plan is located on your personal Healthy Decisions Wellness account. This is an on-line format. Log-into your personal account at www.cmmfhealthydecisions.com, when you open to your account, scroll half way down the page, it is located in the middle of the page. Select your goal, readiness to change and strategies to attain your goal.
Q. Do I have to schedule a one-on-one with a Healthy Decisions Counselor?
A. Health counseling requirements are based on results of your WebScore HRA. Based on results, participants will be notified of risk groups and deadlines associated, specific to their individual risk by February 17, 2012. Participant information is specific to each individual, based on individual needs. If you have questions, please contact Healthy Decisions.
Q. What if I do not get a letter from Healthy Decisions by February 17, 2012?
A. If you have not received your program information, it is your responsibility to contact Healthy Decisions for your program information.
Q. How do I schedule my Healthy Decisions counseling appointment?
A. You will need to log into your personal Healthy Decisions Wellness page at www.cmmfhealthydecisions.com, go to your personal account, and click on the link to schedule your appointment
Q. What are the Healthy Decisions Wellness Activities (must complete 5)?
A. Healthy Decisions Wellness Activities are activities related to preventative screenings (mammogram, colonoscopy, annual PCP visit, etc). The Program requires completion of 5 activities, which individuals may select.
Q. Do I need to be enrolled in CMHC's medical insurance to participate in the wellness plan?
A. Yes, the Wellness Program is available to benefit eligible employees who enroll in wellness as part of their medical benefit plan.
Q. What is the reimbursement program from which I can receive up to $125?
A. This is the reimbursement program-Healthy Decisions Advantage. By participating in health/wellness related activities, you can receive up to $125 reimbursement for your participation.
Q. How do I submit reimbursement information?
A. For approved programs (please refer to the Healthy Decisions website www.cmmfhealthydecisions.com for approved programs), send a copy (inter-office) of your receipt and program attendance to: ATTN: Jill Brayman, Healthy Decisions.
Q. What happens if I do not complete a required program component?
A. By actively participating in the wellness program maintains your eligibility to receive the lower medical wellness premium. By not completing the required steps, you will be ineligible to continue to receive the reduced medical wellness premium.
