You are in the right place! Virtually all the information you need to help you complete your enrollment form can be found on these two valuable resources:

Visit the Healthy Decisions section for additional information as well as program deadlines. Visit the Chronic Condition Health Management section for additional information about the program as well as new participant packet information.

4. Find a Doctor - If you are looking for a PCP fill complete the Find A Provider Form and someone will contact you with an avialable provider.

5. YouTube Enrollment Videos:

The YouTube video will fully explain how to complete your enrollment form. The Enrollment booklet will illustrate coverage. Additionally, at the bottom right of this screen is a list of the Powerpoint presentations.

As an additional alternative, you will notice each Powerpoint presentation has been prepared as a YouTube video. For example, many employees find that clicking on the YouTube video about the Wellness Plan is the easiest way to understand how the program works and the participation requirements.

A quick word on the medical insurance:  Given the complexity of medical insurance and recognizing medical premiums are often the largest deductions out of paychecks, five different YouTube videos have been prepared to explain medical fully.
